编者按:中国肿瘤免疫及靶向治疗高峰论坛暨首届高博医学论坛于4月20-21日北京举行，秉持“精准，整合，创新”的理念，围绕肿瘤的精准诊断，免疫及靶向治疗，基因治疗，造血干细胞移植等前沿话题，开创免疫研究领域先河，邀请海内外500余位专家参与盛会。被誉为全球患者“最后能求助的法庭，医学诊断的最高法院”的梅奥医疗集团，本次会议特别邀请了美国梅奥实验室医学和病理部陈东博士，与参会医生探讨血液肿瘤的伴随诊断，以及各类检测手段在血液病进程中所起到的关键作用，同时强调了准确诊断的重要性。《肿瘤瞭望》也专程邀请陈东博士分享在不断变革的检验环境中临床实践经验。
《肿瘤瞭望》：请您介绍在日常临床实践工作中，如何诊断血液恶性肿瘤？Q1：In daily clinical practice， how do doctors at Mayo Clinic diagnose hematological malignancies?
陈东教授：随着检验技术等不断革新，在血液病的精准分型、预后分层、个体化治疗和复发监测等各个方向给予辅助指导，提升血液病的临床诊断水平。梅奥实验室执行的伴随诊断检测主要由参考实验室或制药公司的CLIA认证实验室执行。 血液学专家将进行骨髓活检并将样本送到当地实验室进行初步评估。自动化病例检验流程：由收取标本开始，再到打上条形码，做筛查有无漏掉的标本，观察标本的日期，取量是否合适等准备工作后，便到了样品分类，70%的标本可以在传送带上通过机器分类带到相应的实验室，再检测得出结果，基本实现自动化、高效率工作流程。此外，中心实验室的validation也很关键，例如如果患者确认为AML，则将骨髓抽取样品送至参考实验室进行进一步测试，为进一步明确诊断和分型。
With the advent of new technologies in clinical laboratory testing， Hematopathology practice has made significant progresses in precision diagnoses and classifications of hematologic diseases， prognosis prediction， personalized treatment determination and remission/relapse monitoring. ?The companion diagnostic tests are mostly performed by reference laboratoriers， CLIA certified laboratories of pharmaceutical companies. Bone marrow biopsies and aspirate are sent to local laboratories for an intial assessment. If AML or other hematologic diseases is confirm， bone marrow aspirate samples are sent to the reference laboratories for further testing.
《肿瘤瞭望》：您能否从基础研究到临床实践的角度给我们介绍一下血液恶性肿瘤的靶向治疗？Q2：Could you please introduce the targeted therapy of hematological malignancies from basic research to clinical practice?
陈东教授： 在基础研究中实验室通常采用细胞培养或动物模型作为分子机制的研究方法。 一旦在临床研究中发现了潜在目标，先会在基础实验室进行抗癌细胞毒性研究，使用细胞培养和动物模型确认药物有效性。 根据药品的毒性特征，进行了I期临床的研究。 如果没有观察到不良反应，可以根据试验设计进行II期和III期临床研究。
Molecular mechanisms are often studied in basic research laboratories using either cell culture or animal models. Once a potential target is identified by clinical study， the basic laboratories will conduct anti-cancer cell toxicity studies to confirm the drug effectiveness using both cell culture and animal models. Based on chemicals’ toxicity profiles， phase 1 studies are conducted. If no major adverse effects are observed， phase II and III studies can be conducted based on the designs of trials. The data will be collected and analyzed to assess the efficacy of the new drugs， companion diagnostic tests and side effects for potential FDA approval.
